What Decision Governance Means
Decision Governance is the discipline of establishing who can authorize AI-influenced decisions before they produce consequences. It is not model governance. It is not risk management. It is not compliance prep.
The question at the center of this work: when AI has a hand in a decision, who says it was authorized to move forward?
Most enterprises cannot answer that question. AI produces output. Someone acts on it. Nothing in between confirms the decision was authorized. Decision Governance closes that gap.
Three requirements must be in place before a decision moves: scope, alignment, and sign-off. Miss any one and governance is theater, not fact.
Scope
The AI was authorized to be involved in this type of decision. It did not exceed what it was permitted to do.
Alignment
The decision follows the rules the organization has established — the policies, thresholds, and review requirements currently in force.
Sign-Off
The right person — with the authority level this decision requires — has reviewed it and taken ownership before it moves.

Why Runtime Authority Matters
You cannot establish authority over a decision after the consequences are already in motion. A review after the fact can explain what happened. It cannot authorize something that already moved.
Runtime authority is the active structure in place at the moment a decision is about to move. It operates before the decision takes effect. It answers whether the full chain — AI output, review, sign-off — was authorized to go forward.
There is a real difference between establishing authority before a decision moves and auditing what happened after. Audit systems record what occurred. Authority systems determine what was allowed. Organizations that have only the former know what went wrong. They cannot prove what was permitted.
The Execution Gap
The Execution Gap is the space between AI output and organizational action where no authority exists. It is not a technology failure. It is a design failure — what happens when an organization deploys AI without building the layer that determines what those systems are allowed to do.
Without runtime authority, policy is intent. With it, policy is enforcement. The difference is between believing you govern AI work and being able to prove it.
03 · The Visibility Problem
Why Visibility Is Insufficient
Most enterprise AI governance investment has gone into visibility tools — system inventories, risk registers, policy libraries, monitoring dashboards. These are worth having. They tell you where AI lives, who owns it, and what risks have been reviewed.
They do not tell you whether a decision shaped by AI was authorized before it moved.
Visibility answers the question: does AI exist here? Authority answers the question: was AI allowed to do this? These are related questions. They are not the same question.
An enterprise that knows AI is in its claims processing, underwriting, customer remediation, and routing — but cannot prove those decisions were authorized at the moment they moved — has visibility without governance. This describes most enterprise AI deployments today.

04 · Control Failure
Why Human in the Loop Is Incomplete
"Human in the loop" is how most enterprises respond to AI governance risk. A human reviews the output. A manager approves the recommendation. A committee sees the decision. The assumption is that the human's presence is the control.
Presence and authority are not the same thing. A human in the loop can be the wrong human — reviewing under an authority level that does not match the stakes, relying on AI output without understanding how AI shaped the decision, approving under time pressure. Being in the loop does not mean you had the right to authorize what moved forward.
Human oversight becomes enforceable control only when authority is named before the decision moves — the right person, at the right level, with the right information, and a clear mandate to own what comes next. A signature or a click is not enough. Governance is not about presence. It is about accountability established before the fact.
Presence Is Not Authorization
A human in the loop can still be the wrong human — working with incomplete information, approving something that exceeded their actual authority, or moving forward without leaving a record that would hold up under review. Having a human present is not governance. Governance is knowing the right person reviewed it, at the right level, before it moved.
05 · Governing Object
Why the Decision Path Becomes the Governed Object
Enterprise AI governance has focused most of its energy on the model itself — development, testing, monitoring, and risk classification. The work matters. It also misses the point.
The AI model is a participant in the decision. It is not the decision. The decision is what carries consequences. The decision is what needs demonstrable authority behind it when something goes wrong and someone comes looking.
Governing the model does not govern the decision. A well-governed model operating inside an ungoverned process still produces an ungoverned outcome. Consequences attach to the decision, not to the model that had a hand in it.
The full chain — AI output, human review, escalation, final sign-off — is what Decision Governance addresses. Every step in that chain has authority requirements that governing the model alone will never satisfy.

Why Enterprises Need Authority Before AI Influenced Execution
Every serious organizational decision already has authority requirements built in — approval levels, supervisory review, documented sign-off. These requirements exist because organizations have learned, at real cost, that consequential work cannot run on informal confidence.
AI-influenced decisions do not inherit authority by default. When AI shapes a recommendation, a classification, or a routing call, the person who acts on that output is still exercising authority — over a decision that AI helped create.
